Enclosure slip template

Copy this file to your hard drive to print three enclosure slips to a page using the following instructions:
1) Do not try to open the template!
2) Place curser over the link and click the right mouse button.
3) Select "Save link as" and save the file to a disk or hard drive.
4) Open the file in Adobe Acrobat and type in address information relevant to your center.

Note: TIFF files are for use for printed pieces that you produce in-house, EPS files are used for print pieces created by outside vendors; JPEG files are for use on the Web. For best resolution when resizing, please choose the logo closest to the size for your printed piece or Web site. If your downloaded image is not of good quality, please contact Communication Services for assistance.
