During the course of your graduate program at NC State you will be required to complete a variety of steps involving paperwork in order to obtain your degree. The forms and the timelines for completing these steps are found below:

• Graduate School Application Forms


• Residency Establishment Forms (click HERE for more information)


• W-4 Form (Federal Withholding Allowance Certificate) - complete upon arrival


• I-9 (Employee Eligibility Form) - complete upon arrival


• Selective Service Registration Compliance Form - complete upon arrival


• Patent Agreement Form (click HERE for an explanation) - complete upon arrival
  
• Plan of Work (POW) Forms (required of new students in their second semester)
   
• Annual Report Forms
  
  All students are required to meet with their Graduate Advisory Committee at least once a year and must prepare an Annual Progress Report. This report must be signed by the student and each member of his/her Graduate Advisory Committee.
  
  • Deadline: May 31 in each year of enrollment.

• Master's Students - Request for Permit to Schedule Master's Oral Exam (min. 3 weeks prior to proposed date of exam during the semester of graduation)
  
  
• PhD Students
  Request for Approval to Schedule Ph.D. Preliminary Exam (min. 2 weeks prior to the proposed date of the examination)
  
  Request for Approval to Schedule Ph.D. Oral Exam (min. 2 weeks prior to the proposed date of examination and no earlier than 4 calendar months after successful completion of preliminary exam)
  


• All Graduating Students
  Diploma Request Card (no later than the end of the sixth week of semester of graduation)
  
  Electronic Submission of your Thesis or Dissertation  (all graduate students)
  (four weeks prior to last day of classes in the semester of graduation)
