All Course Action Forms must be submitted in MS Word Format. No exceptions.
The Approval Process
Please use the new CAF with the accompanying instructions . Each CAF must be accompanied by a syllabus conforming to http://www.ncsu.edu/policies/academic_affairs/courses_undergrad/REG02.20.7.php

Attendance, Honor Code and Disabilities
The Graduate School requires links to university policies on these three issues. There is some leeway on the wording of the course attendance and honor code policies, depending upon how these are to be applied in the individual course; however, deviations of the wording of the disabilities statement are probably not a good idea.
Student Learning Objectives
Another item that causes faculty trouble is the construction of appropriate Student Learning Objectives. Simply put, Student Learning Objectives are measurable outcomes you expect students completing your course to achieve. If an objective is not measurable, it is not appropriate. Likewise, if it is not written in terms of an expectation, it is not appropriate. Some examples of Student Learning Objectives are provided here
