How to use the NCSU Plant Disease and Insect Clinic Database

If you already have a Unity (NCSU) computer account ID:

Logging in

  1. Open your Firefox browser. You can also use Internet Explorer but the database is optimized for Firefox.
  2. Type in https://plantclinic.ces.ncsu.edu
  3. Depending on your computer set up, you may see a security warning pop-up. This warns you that our website contains both secure and nonsecure content. Type "yes" if the security screen comes up.
  4. The NCSU WRAP Login screen comes up: Login with ID and Password. Your login is your Unity ID and password.

  5. After you log in, the PDIC Main Menu screen opens.

From this menu you can:

    • Enter new sample information: Click "New" in the My Samples box.
    • Check on samples submitted
    • Check on samples in progress
    • Check on completed samples
    • Change your password
    • Search for address

Throughout the database, fields with a GREEN DOT are required fields. Words in Green are links to other tools. The database uses several other icons to help you with navigation

Sample Entry

  1. Client: The name of the owner of the plant. Type in at least 4 letters and then select the desired name from drop down box. If the person's name does not appear when you start typing, type an asterisk after the name. All matching names in our database will appear.

  2. If the name does not appear:

    • Right click "Create a New Address Book Entry"
    • Select "Open in a New Tab or New Window."
    • Fill out and save the information.
    • Navigate back to the "Sample Entry" screen to complete the sample entry.
    • If you provide an email address for your client, an account will be automatically be created and he or she will be sent a welcome message. This allows clients to log in to view their own reports. This is not possible without an email address.
  3. Agent: If you are a County Agent, your name will automatically appear.
  4. Consultant and Other: Consultant should be used for Landscapers, Pest Control Operators, etc. Other can be anyone else involved with this sample that needs a copy of the report. Leave this field blank if possible.
  5. Send Invoice to: Select "Client" unless someone else will be responsible for the bill.

Location

  1. Host Site Type: Select the best description for the location where the plant was growing - field, commercial, home garden, etc.
  2. Host Address: The address should be "Same as Client" in most cases. If the sample came from a different location than that given for the client, click "Not Listed (let me type it in)" from the drop down and fill in the location. This is useful if the sample came from a second home or the home address is not near the place where the plant was collected.

General, Problem Type and Sample Type

  1. Date Collected: Select the date by clicking the calendar icon. Check "Approximate" box if you're not sure of the exact date collected.
  2. HOST: Type in the common name or scientific name of the host and select the correct choice from the drop down list. Note: Typing in four (4) letters of the name will start the search and generate the drop down. You can use any part of the name you know. Click on the name you want to bring it up into the box. If you do not know the host or the host name is not found in the list, select the button "I Don't Know" or "Not Found on List". In this case tell us what you know about the plant or situation (e.g. Interior Molds) in the comments field
  3. Variety: Note the variety or cultivar, if it is known.
  4. Problem Type: Select the type of problem, if known, from the drop down. In most cases, click "unsure" unless you are submitting an insect only.
  5. Sample Type: Be sure to check the box "Check this box if a physical sample will be sent to the address below" if you are sending a physical sample.

File Attachments

  1. Click on the "Choose File" button to add an image. Browse to any images you want to upload and click to select.
  2. Include a description of the picture in the "Label" box.

Disease Information

  1. Click the dropdown and select percentage of the crop, area, or planting affected.
  2. Symptoms, Distribution, and Parts Affected: select all that apply. Additional information that doesn't fit here goes in the "Comments" box.

Enter Insect Information

  1. If this is an insect sample, include additional information here.
  2. Be sure to tell what the insect was doing and an estimate of how many per plant or how many found in this situation.

Other Information

  1. Include any other information you might have about the sample. Be sure to include specific information on site history, pesticide/ herbicide applications, and any control recommendations you need (e.g. organic control recommendations).
  2. When you are finished, click the "Submit" button.
  3. Click on "Summary Report." Print this page and include a copy in the box with the sample.

Guest Users

  1. Open your Firefox browser. You can also use Internet Explorer but the database is optimized for Firefox.

  4. The NCSU WRAP Login screen appears:

  5. Click the dropdown, select "Guest Access", and click "Continue Login." The PDIC Online Welcome screen appears. If you got a welcome email, use that information to log in.
  6. Click "Create a New Account" and fill out the address information, including a valid email address.
  7. A confirmation email will be sent to your email address. Reply to confirm and login with the ID and password included in the email. You can change this password at the Main Menu after you log in for the first time.
  8. The PDIC Main Menu screen opens: From this menu you can:
    • Check on samples submitted
    • Change your password
    • View your invoices and payment history
    • Search for addresses you entered. Click "New" to create a new sample.

Sample Entry

  1. Client: If you registered as a client your name will automatically appear here. If, not please enter the client name. A client is required for all samples (must include city, county, state and zip code).
  2. Agent: Agent is reserved for county agents. Add your county extension agent name ONLY if you are working with the agent on this problem and they are expecting a report for your sample.
  3. Consultant: If you registered as a consultant, your name will automatically appear here.
  4. Other: Enter the name of anyone else who needs a copy of your report. Leave blank if possible.
  5. Send Invoice to: Select "Client or Consultant as needed" Sample collected by: Select "Client or Consultant"

Completing the sample form

Continue filling out the sample information (location, general, problem type, sample type, etc.) as described above for Unity users.
